Background

PointClickCare supports the export of electronic health information (EHI).

As of Version 4, PointClickCare’s Health IT modules, and only the modules listed below, are ONC 2015 Edition compliant. The ONC HIT Certification Program is a voluntary certification program established by the Office of the National Coordinator for Health IT to provide for the certification of health IT. The program supports initiatives such as Promoting Interoperability Programs (previously Medicare and Medicaid EHR Incentive Programs) used by eligible providers and hospitals for funding and the 21st Century Cures Act. The use of certified health IT has expanded to other government and non-government programs. Reference to the certification and a list of PointClickCare’s current certified modules can be found here.

Format

The export artifacts are encrypted compressed files. A single artifact is generated per patient for single export, or an artifact can contain multiple patients for population export. Please consult your PointClickCare documentation on obtaining the encryption keys.

Please note, certain data elements in the patient file may include references to other records. Using the ID listed in the reference, further details on the referenced records can be found in the Index files.

For example, under the Care Team data element, there could be references to Practitioner, PractitionerRole, Organization or Related Person as seen below:

{"resourceType":"CareTeam","id":"a080fbfe-a58a-4bc0-9504-6c1ce50304c0-17107600","identifier":[{"system":"http://www.pointclickcare.com/careTeam/identifier","value":"a080fbfe-a58a-4bc0-9504-6c1ce50304c0-17107600"}],"status":"active","subject":{"reference":"Patient/a080fbfe-a58a-4bc0-9504-6c1ce50304c0-17107600"},"participant":[{"role":[{"coding":[{"system":"http://nucc.org/provider-taxonomy","code":"207RG0300X"}],"text":"Allopathic & Osteopathic Physicians : Internal Medicine : Geriatric Medicine"}],"member":{"reference":"Practitioner/a080fbfe-a58a-4bc0-9504-6c1ce50304c0-26606-5"}},{"role":[{"coding":[{"system":"http://nucc.org/provider-taxonomy","code":"207RG0300X"}],"text":"Allopathic & Osteopathic Physicians : Internal Medicine : Geriatric Medicine"}],"member":{"reference":"PractitionerRole/a080fbfe-a58a-4bc0-9504-6c1ce50304c0-26606-5"}},{"role":[{"coding":[{"system":"http://www.pointclickcare.com/careTeam/role","code":"Sister-in-law"}],"text":"Sister-in-law"}],"member":{"reference":"RelatedPerson/a080fbfe-a58a-4bc0-9504-6c1ce50304c0-1543640-17107600"}}],"managingOrganization":[{"reference":"Organization/a080fbfe-a58a-4bc0-9504-6c1ce50304c0"}]}

A corresponding data element with the id reference can then be found in the index files:

{"resourceType":"Practitioner","id":"a080fbfe-a58a-4bc0-9504-6c1ce50304c0-26606-5","identifier":[{"system":"http://www.pointclickcare.com/practitioner/identifier","value":"a080fbfe-a58a-4bc0-9504-6c1ce50304c0-26606-5"},{"system":"http://hl7.org/fhir/sid/us-npi","value":"1477519569"}],"active":false,"name":[{"use":"official","family":"Good","given":["Jordan"]}],"telecom":[{"system":"phone","value":"(145)354-5902","use":"work"}],"gender":"male"}

Additionally, if there are Consents and Advanced Directive documents attached to a patient chart, they will be available through the 'Files' folder. Assessments will be available through two separate folders containing MDS and non-MDS Assessments.

Radiology images and laboratory results attached to a patient chart are available through the 'radiology-attachments' and 'lab-attachments' folder.

EHI is given as both standard FHIR R4 and non-FHIR custom resources. Custom resources conform to the basic requirements for any FHIR Base Resource with id and meta elements. Data is output using Newline Delimited JSON formatted files or Comma-Separated Values (CSV).
